At the time of this writing, Toto's FAQ page states Behavioral Therapy is covered under base policy, whereas the sample policies state that a optional addon is necessary.

Pre-existing Conditions Policy Pre-existing conditions are not covered.

However in some states, some conditions may be considered curable. If your pet doesn't show signs for these conditions for at least 365 days, they may cover it.

According to their sample policy on their website, pre-existing conditions are "...any Illness or Injury which occurred, reoccurred, existed or showed Symptoms, whether or not diagnosed by a Veterinarian, prior to the Pet Original Start Date, Coverage Period or during the Waiting Period."

Hereditary & Congenital Conditions Coverage Not Clear

At the time of this writing, the home page states there is a 365 day waiting period for hereditary conditions, but it says 30 days in sample policy and in FAQ section of the website.

In the sample policy it also states that Congenital and Hereditary coverage is an Add On. Whereas the website states it is included in the Base Policy.
Continual Coverage for Chronic Conditions [INCLUDED] Continual coverage for chronic conditions is included with the policy.

Chronic Conditions Policy Chronic conditions are covered as long as they are not pre-existing and have not occurred during the waiting period.

There is a waiting period for chronic conditions that they deem are hereditary/congenital. Based on information on their website, this includes but is not limited to: heart disease, diabetes and arthritis. These diseases can be hereditary/congenital in origin but there can be other causes of origin as well. For example: arthritis can also be caused by infection of the joint or trauma to the joint. (See 'Hereditary and Congenital Conditions Policy" above.)

Bilateral Conditions Policy If a bilateral condition happens on one side before they policy starts or during the waiting period, that condition will not be covered if it happens on the other side. Even if it happens after the waiting period.

The way it is written in their policy it appears this applies to all bilateral conditions (e.g. eye problems, ear problems, knee problems, hip problems, etc)

How is Reimbursement Determined In the sample policy it states that reimbursements are based on UCR (reasonable and customary).
Their definition of Reasonable and Customary charges: "Reasonable and Customary Charges means typical fees or the cost that Veterinarians charge in Your geographic area based on available veterinary fee information and proprietary data."

Reimbursements are calculated by subtracting the deductible before applying the reimbursement percentage.
